Visa Forms
Effective immediately, the Visitor, Student and Worker application forms that will be accepted by Citizenship and Immigration Canada will be the new Application Forms enclosed in this website (IMM 5257, IMM 1294, IMM1295). To ensure you have the most recent application form, please download the application forms from our website and discard any previous downloads. Once the application is completed, click on the “Validate” button located at the top or bottom of the form and print your application form. This will generate a barcode page (page 5 of 5). When submitting your application, to ensure your encoded data is captured, you must include the last page (page 5 of 5) which contains your unique barcodes.
Note (For New Form): each individual applicant must complete and sign the new application form, even if individuals are travelling as a family. Applicants who are under the age of 18 must have their form signed by a parent or a guardian.

a. Each applicant must submit a separate application form.
b. Each Adult applicant above 18 years of age must submit a family information form and education & employment form.
c. Each Applicant under 18 years of age, if not accompanied by any adult family member, must also submit a family information form and education and employment form.

2. When the form opens, save the form on
your computer.

2. Select Save Target As from the

3. Save the file on your computer.

| As a federal government
department, the Immigration and Visa section is bound by the Privacy Act not to
discuss the details of any case with anyone except the applicant and certain
authorized persons. Before the High Commission or Consulate General authorities
can release information, the applicant must provide us with specific written
permission to do so. Only a Canadian citizen or permanent resident or temporary
resident in Canada can obtain this information. |
